When you request an appointment through this website we collect the information you enter in the booking form: your name, phone number, email address, draw location, preferred date and time, the ordering provider, the service you need, and any notes you add. We collect this only so we can contact you and carry out the collection you requested.
Form submissions are delivered to High Point Sticks through our website hosting provider, Netlify, and are accessed only by our staff. We do not sell or rent your information, and we do not share it with anyone other than the laboratory and ordering provider involved in your care, or as required by law.
Please do not include detailed medical history, insurance numbers, or other sensitive identifiers in the booking form. If we need that information we will ask for it by phone or in person during the visit.
This website does not use advertising cookies or third-party tracking. It loads the Inter typeface from Google Fonts, which may record standard server logs such as your IP address.
You may ask us to correct or delete the information you submitted at any time by calling 940-532-1313.
